How to add a feeback button in MS-OUTLOOK email ?

I want to add a graphical interface within the ms-outlook mail in my signature thus to ask for a feedback from client either bad,good or excellent with a comment box by which we got a feeback from the client via an automated mail on successful completion of the dialogue box.
I want to know the procedure to implement in my regular client mail.
