Read ActiveX Textbox from PDF in C#

I have Designed Simple form in MS Office 2007 in which i have 2 textbox from activeX control for FirstName and LastName.

Then I convert this Doc file into PDF file and Now i want to read this ActiveX control from Converted PDF file in C# and Insert Data from Database.

How can i do this.
