The problem is in your datasource assigned with the crystal report during crystal report designing.
To overcome this issue you can try this -

1. Add a datasource. During this process it ask you to save the connection string in your project, Click yes. By this the username and password will save on app.config file as well as in your application setting.
2. Use this datasource to design your crystal report. In crystal report wizard it will ask for for datasource. Your datasource is saved on project datasource, choose it.

If this solution not works for you please let me know an alternate option is also available.
