Max Execution time error despite safe_mode off and value set to 0 in php

I am getting the following error:

Fatal error: Maximum execution time of 30 seconds exceeded in /var/www/html/...file.php on line 192
Even though the max_execution_time is set to 0 and safe_mode is set to off in php.ini Where else can the execution time limit be specified? (like httpd.conf or htaccess files)? What are recommended steps for the RCA?

Also where is the maximum execution time for mysql query defined?
