Multi columns queries on list box

I have a table 'association' containing columns (category, sub_cate, priority)

and I have separate tables for category and sub_category.

now I want to create a list box that displays all category and sub category from their respective tables and there will be a drop down list against each option of list box, that will have 1-10 values for set priority of each category and sub_category.

and then I want to save these 3 things in my 'association' table.

plz help me through this.. thanx !!
