I want to do a drop down menu on my custom form.

How do I take this form on http://wodco.uk/promo/ and change the "last name" section into a dropdown list that matches the form style? I have edited the form on my end obviously to suite my use and for my use I need the drop down to display product types for example:

Half Sheet
8x11
PromoProduct
T-shirt


Any ideas on how to get that working for the form located on the top of that page?
