"Projects fail due to lack of planning". Yup. Agile or no.

The other dead-on observation, which I've seen: "...the iterative process may lead to iteratively improving, one failure at a time, towards the wrong outcome."
I've yet to see a genuine agile project actually work. The ones that seem to have worked have so much planning and upfront design that it really wasn't agile, even if they used all the words and pretended otherwise.

In my current project I was given one line requirement topics and asked to estimate development effort. Why ? because apparently it is an "Agile" project

The only ones on the project who actually know what agile is and isn't. 
My company is the same way. "We're an agile shop! So here is your project budget, here is the list of deliverables, and here is your delivery date. Now go be agile!"
