How do I take a multi-stage from from my website and package it up to put on other websites?

I have a multi-state from I spend a decent amount on money developing and now I want to put the same form on other websites I have but when copy and paste the html it shoes the form but the steps are not separated and they run into each other. This is coded in html, php to my knowledge. Any help or starting points would be great.
