Hello Everyone,
I am developing an application for hospital using VB.Net 2008 and SQL Server 2005. with around 2,00,000 patients and atleast 3,000 bills per month.
The system consists of around 100 machine connected to a windows server(2010)
The application is not connected to internet.
Will there be any considerable change in perfomance of the application if Wcf services are used

No, you probably won't notice any differences that can be traced to WCF (of course this completely depends on your skills as a programmer, too).

Also, seriously consider using SQL Server 2008 as opposed to 2005. It provides encryption of your data (where 2005 does NOT). Patient data MUST BE PROTECTED, and SQL 2008 is a better choice in this regard.
