these are my text boxes which are in my website.i want to store these fields data in my database.database in on my hosting
my database name is
"mis-db"
and fields of my database are
name,address,contact number,email,subject,comments
when user click on submit button these fields data must store in my database,
and after that my text boxes must be empty for next user,or next comments

but i don't know how can i perform this task.so please help out me.

which server side scripting language you used. php,asp.net or JSP
