creating online survey genration form in asp.net

i am currently working on a project which is a web based project can be used for survey genration like :

i am not getting any clear idea how to do this i am new to asp.net and i select this project just to learn asp.net.]so plz can any one share some helpfull tutorial for this type of work.

It does not work this way.. If you are new to asp.net, then you should learn the basics of asp.net first. Once you are comfortable with it, then you can go for the implementation of a project
