How to code without thinking?

Use of a narrow-focus strategy in test-driven-development.
While observing different teams and individual developers failing to establish a test-driven development process, I follow a TDD recipe that works fine for me since a coupe of years.  In this article I outline possible reasons why TDD doesn't work (when it doesn't) and suggest a step-by-step algorithm that has led me to using TDD as a natural software development approach.
