How to take submit data from a Webpage to a MSSQL db?

I am very green to programing and am attempting to make a simple Webpage that someone can enter a few pieces of information, such as name, age and sex via textbox and radio button. Can someone provide me code will connect to a DB (hosted on my local machine) and insert the data that was enter by the user after they click a "Submit" button. Thanks in advance.
