how creating Installation file in c# desktop application...?

I have develop an desktop application with database connectivity.
database will be in SQL-Server 2008,

My application is done and working well.

so, i think to create an installation file for this, i want to create a custom form for installation wizard that will guide user for installation steps and also ask for license key.

and main thing is that at the time of installation the database will created on users system using SQL script at run time.

Please help me...!!!!!
