The "extra" means that the affair was with someone outside his marriage, to make perfectly clear it wasn't an intra-maritial affair, which is uh ... when you ... err... you know ...

In fact, what the BBC World Service announcer said this morning was "an extra {pause} maritalaffair" rather than "an extramarital {pause} affair".
