Read user inputs in form fields from a PDF

Hi,

Could you please guide me to read user inputs from an editable PDF. For instance please refer a sample PDF available in URL : http://www.irs.gov/pub/irs-access/f1065sm3_accessible.pdf[^]

User can fill this PDF as save it back, where my application has to read user inputs from the PDF.
