MDF is the extension for all SQL database files, it does not matter if it attached to some remote SQL server or you have it locally (or even embedded)...
When you directly attach your application to some local MDF file you are using an embedded version of the SQL server engine (that have some limitations). This kind of usage is good for small databases, in which case you do not want the user to install the whole SQL server pack, but you provide him with an embedded version during installation...
