Component for dynamic print template designing

Hi,

I have print template fields Invoice,qty,Discount,VAT,Gross Amount, Net Amount,Unit Price, Quantity like this. Now i want to make this template on user choice which fields he required only has to generate a report. Any one has idea please share to me.

Display a form with all the options and let the user select the ones he/she wants by clicking checkboxes, radio buttons or similar. Then add or remove items from your template as necessary.
