Employment Agreement      [rbtnAgreementYes]  [rbtnAgreementNo]  [rbtnAgreementNot]

Those three are radiobuttons. In this way I created my design page.
In the sameway there are some more titles and radiobuttons please tell me how to create the table and storedprocedures.

Some sample code how to check those radiobuttons in c# code would help.

Thanks, perhaps the OP should be reminded to stick to the first post and improve it if necessary but I don't know what's the prefferred way at the moment. Years ago posting a new thread for the same issue wasn't well tolerated but is it different nowadays with Q&A?
