Problem solved, but I leave this here in case it helps anyone else. I didn't see the tiny <v> button on the formatting toolbar. This is the same as the var button in this editor.

Up until yesterday there were less than a 1000 votes, with most of the choices around 56 - 60%.
As of today the votes are more than doubled and the distribution changed dramatically, pushing "I am developing software not using any UI" to more than 60% and pushing the others around 20%.
Did something happen? It's fairly strange to see such an drastic increase of votes in a short time near the end of the week...

Yeah - someone did some ballot stuffing.
We deliberately keep voting for the polls very open to allow as many people to vote as possible, but I guess some people just enjoy ruining things for others.

Ballot stuffing a poll on CP is mostly an exercise in futility... and doing that in a place full of developers is also very silly.

When I use the "Copy Code" function of a code block in a message or an article and paste it into my IDE special characters show as their HTML-escaped version, e.g. '<' gets pasted as "<".

I used to be able to up-vote on other's comments to surveys. I am not able to do that anymore. What is interesting though, is that someone up-voted one of my comments, yet I don't see it listed in the bottom right of the comment.
Has this feature been removed, or is this a bug? If it has been removed, then how was someone able to up-vote my comment?
Edit: Latest version of Chrome, Windows 7. No ad-blockers.

What browser are you using? I'm on Chrome Version 61.0.3163.100 (Official Build) (64-bit), on Win 10 Version 1703 (OS Build 15063.608).

Same for me with Firefox 56.
Seems to be a Firefox specific problem with the rendering. Comparing the page source here and on the survey page:

However, I have the same version of Chrome as Griff does and both my home machine (win 7) and my work machine (win 10) does not work for me.

See my posted HTML snippet:
The code was there (image, link), but it was not shown here and the only difference was the width (which is sourced by the thread depth according to Chris but may be still the reason).
I'm now at home and have the upvote option. The difference (same FF version):
Windows 10 with 16:9 HD widescreen monitor at home.
Windows 7 with 4:3 monitor at work.
So I reduced the FF window size here: The option is not shown anymore.
Result: Seems to be a rendering problem with small window size.
If you have a small monitor and a sidebar like history or bookmarks, try it without the sidebar.
