I once released a Tetris like game that could be launched with the correct key sequence from anywhere within the application. This went out to well over a hundred servers with 5 to 10 users at most of the installations.
I doubt anyone ever found it.
We also had a help icon on each screen that when clicked popped up a message saying help not implemented. I changed one of them to say RTFM. I know someone found that because they phoned up to complain. I argued this showed we should actually get around to writing the help some day.
