Create dynamic template field inside dynamic gridview

Firstly, i dont intend to create the itemtemplate as a gridview column. But, I want to create 3 rows with 5 columns.
First column will be my system's name, second row is image for each system, and third row is checkbox for user to select.

How can I achieve that?
