Did you check whether the relative path of the image is correct in the final strHTML? Before printing the page, try to show it and check whether it's displaying the image.

I think AddHandler ChatService.OnContactsRequired, AddressOf ChatService_OnContactsRequired line of code should be called before ChatService.Start(). Also check if you are getting any Application_Error

Are you getting any exception? Because in my sample application both the approaches are working just fine! Though you don't need a DataAdapter here. Only this code should work
string activationCode = Guid.NewGuid().ToString();
SqlConnection sqlcon = new SqlConnection(ConfigurationManager.ConnectionStrings["MyConnectionString"].ConnectionString);
using (SqlCommand sqlcmd = new SqlCommand("USP_CandidateActivation"))
{
sqlcmd.CommandType = CommandType.StoredProcedure;
sqlcmd.Parameters.AddWithValue("@UserId", 3);
//sqlcmd.Parameters.AddWithValue("@ActivationCode", activationCode);
sqlcmd.Parameters.Add("@ActivationCode", SqlDbType.UniqueIdentifier).Value = Guid.NewGuid();
sqlcmd.Connection = sqlcon;
sqlcon.Open();
sqlcmd.ExecuteNonQuery();
sqlcon.Close();
}

That means now it's executing the Action successfully, but your data access layer using Entity Framework (EF) is having some problem. It could be EF installation issue! So you have to google this error.

Then what are the columns you want to show in each row? you can work on your data source to split into 2 rows and then bind into the grid!

well, in that case you can create a table which will store UserId and FoodItemId, one row for each food item per user

When I run the code for a *.docx file, it worked fine for me! Please debug the code and check whether the file path is correct. Also you can try using response.WriteFile() instead of response.TransmitFile()

Do you have a predefined list of menu? Like menu1 contains foodItem1, foodItem3, foodItem4; and menu2 contains foodItem2, foodItem3, foodItem5 ... like that? Or any user can choose any food item from the list?

there is no such control in ASP.NET which provides that kind of navigation by default. you have to write your own code for navigation. Please download some demo projects for ListView/GridView, and you will get an idea of how it works. I'm sure you'll find a better idea to implement your requirement!
