First problem solved:
Edit Conditions for SQLLogin1 dialog Behavior (there are 3 tags: Events, Cubscriptions and Conditions): add Disabled action with IS_SQLSERVER_AUTHENTICATION_1=0 condition and Enabled action with IS_SQLSERVER_AUTHENTICATION_1=1.
