How I add Crystal report form in VB.NET 2008

When I click on add new item it doesn't show crystal report form, what I do please help me out from this problem.

Don't SHOUT: All uppercase is considered shouting on the internet, and rude.
I know you can find your shift key - you shouted your way through the subject - use it. All lowercase is considered childish.

Did you try looking under the "Reporting" section? Look in the left hand side panel of the "Add new item" dialog: it is near the bottom.

Are you using the Express edition? If so, then it does not include Crystal reports : see MSDN[^] which says:
"Crystal Reports is not compatible with Visual Studio Express Editions."

Nothing.

Crystal Reports will not integrate with any version of Visual Studio Express edition. It won't work. Not at all. Your only way to do it is to stop using the Express edition and buy a "full" version which includes CR or then use a 30 day trial of CR or buy that as well (depends on which version of VS you get).
