So I noticed there was a warning message while browsing Insider news telling me I needed to confirm my email to post, so I did, got the email, clicked the link, got a confirmation page and then...the message is still there. Have a similar warning in this forum too. (Already tried the obvious, refresh, clear cache and refresh.)
EDIT: Seems to be gone now...either fixed or I was just impatient (the email came basically instantly, guess I figured my account's email status would update as quickly )

It might be nice if, on the page of a question asked, a list of the same-user's other recent posted questions (either Quick Answers/Forum) were shown with date/time (sorted from most recent) and title (as links).
( In Quick Answers, perhaps on the right, above/below the Top Experts lists. In Forums, maybe a popup/hover/???. )
If we see a (very) recent question with the same or similar title, we can check if it is really the same question or not.
I (like most of us, I expect) go through the Quick Answers in the presented order (most recent first). We will not necessarily see that the question is a repeat. And in the Forums, it is even harder to detect cross-posting.

How about requiring new users to click on a confirmation link sent to their email address before allowing them to post?
Lots of other boards do it, and it just might make it enough of a hassle that some spammers won't bother.
The difficult we do right away...

It won't really be any hassle, as this can be easily automated.
It might thwart the ones though that are to stupid to that. So why not?

We already require confirmation on some forums and they still spam.

I marked 3 posts as spam, refreshed the page and noted that they were then then marked as removed (at which point I thought - hey God powers, cool).
I then wandered off to see if there were any other posts worth nuking with my new super powers, when I returned to the Lounge the removed posts were back, slightly advanced time. There were no vote counts.
I then repeated the excercise proving my God powers were in my browser only. I thought you had maybe given select users 1 vote nukability, which may not be a bad idea as some here you can trust not to abuse the ability!

Second sighting. I had also red-flagged the three Lounge spam posts. Came back later to see "Automatically Removed" on all three, returned later again to see they had reappeared .

Take a look to the lower left corner. There are some infos there, one of them is which server are you connected to "Web0x". There are some problems with the synchronisation of the different servers.
I have already seen, that I answer to a thread, go to other place and come back and there are 5 or 6 threads less (where my answer not exist yet), just several F5 or go to another forum and come back get you to another Web0x and then.... voila, all is there (or not)
You can check it with your reputation as well, depending on which one you connect, your reputation is 40 or 50 points less than it should

I have seen you mention something like that in the past, so I actually did make a mental note, that when I saw the messages reappeared, the page came from Web03.
When I returned later and saw them removed again, I tried to refresh a few times, but could not seem to "land on" Web03 again, so I did not research it any further.

Sounds like you're hitting a different server with a different set of messages in its cache. An outstanding bug we have is that each server's cache is not synchronised.

Damn I was hoping for God powers! It is not really an issue to me, I would not have noticed except I was nuking spam, I not interested in instant rep information and rarely follow up a question I have responded to unless asked.

I am using Chrome Version 26.0.1410.12 dev-m on a Win7/64 machine.
This is a fairly frequent, but: intermittent, problem. And, I would judge it "minor," in the sense that when you do post the message any content that has not appeared in the Preview frame is in the message posted.
Omissions of line-breaks are most frequent.
While composing messages, on the Forums, including the Lounge, often the preview of the message appears to "freeze" at some arbitrary point, and never updates. And, some of the content of certain lines, at times is arbitrarily "omitted."
As you would assume, given Heisenberg's "Uncertainty Principle," this effect is not happening now, as I write this message
If this is not a Chrome bug (quite possible: lately the dev versions of Chrome have had really surprising behaviors appear), then I hypothesize (very weakly) that it could be related to the intensity of traffic on CP's servers that handle forums.
Could "live preview updating" be "dampened" (queued ?) by a couple of hundred-thousand people writing messages at the same time ?
But, when the preview update never appears, as does happen, after minutes, and much editing of the message, then I suspect something different.
Right now the only thing that temporarily "disappeared" while writing this message from the preview was one line-break, and the question-mark after the line ending: "the same time." Those both re-appeared fairly quickly.
And, in writing the final paragraph here: "final paragraph" first appeared in the preview as "final para." And, now, that's gone.
A "tempest in a teapot" ?
Whoops, the preview just left out the word "teapot:" for about ten seconds.

A few of things could be happening
1. A javascript issue. If you hit F12 to open Dev tools then there's a console that shows errors. A JS error can kill javascript for the entire page. It shouldn't, it can though.
2. Network latency between you and our server
3. Site load, as you theorised
4. A site update in progress. This can really kill the preview.
