Hi...

I want to take survey on product. i need to generate number of question and answer on product. so that i conduct the survey for that product.

any one help me.

example... i say product pen. no of questions 5.

shroud be display ..

question 1: here it will be text-box for the question?

answer field should be. check-box or radio button or text-box.

and place previous and next button to go next question and previous question.

so on..

while taking survey ..

we need to display question and answer type like text-box or radio button or text-box.

Full source available, shows how to create surveys with custom answers etc. You can configure answers to have various options, such as textbox or radio button.
