Oops! Sorry, I read it as 'urgent' yet we would not be able to help here.

It does not work like this here. There is no quick question here. This sounds like your college project/assignment, you should put some effort.

We expect you to put some time in trying the issue that you are facing and then some time in formulating the question while posting here.

Here is what is expected by enquirers:
1. TRY first what you want to do!
2. Formulate what was done by you that looks like an issue/not working.

Sorry but I can't... I don't understand properly your question. What I do understand is that you would like to make a query that would give you some values and from those values you would like to mount a tree with some checkboxes. And after having created the tree you'd like to be able to change some of the checkboxes and then reflect the changes into the database... Is that right? and could you tell me what you've tried so far till now?
