Seriously it's a basic & frequent question. Just Google is enough for you(You can get a good link in 1st page).

Working with MS-Office Access Database(.mdb) and C#.[^]

Try Connection strings for Access[^] for getting help to connect application to Access database and for design Form and making Databased application you need to follow Abhnav and Marksuggestion

You are asking something basic - it might be useful for you thus to start by reading a good book on programming. There are then samples and quickstart videos on msdn that you can go through.
