Extending the ASP.NET CreateUserWizard Control

I am using membership controlls to implement Authantication and authorization. I have enabled my own DB in SQL Server 2008 for membership Controlls by runnig aspnet_regsql utility.
By Default it has username,Email,password,confirm passord ....
My Boss has asked me to add some extra fields like Phone No, Address in existing membership Controlls.
What type of Changes should I made to Database tables and at front End to implement the goal.
Please help me in this regards.
