How to add combination of controls in web setup project custom action page

Hello,

I am using Web Setup Project in Visual Studio 2010. I want add a combination of controls (like radio button, check box and textbox) in the same page. I tried so many times. But i tired to get the solution. In a single installer custom action page i can add only one control. so please help me.
