How to create a \layouts\new.aspx page for creating a custom document library in sharepoint 2010

I want a custom new.aspx page for creating a custom document library and it works fine for me. The problem is that, in sharepoint 2010, I want to show this page in the Create dialog when selecting the customdocumentlibrary icon. When I Click the icon it only shows a create button and navigates to the custom new.aspx page. I want to show the TextBox for giving list name and 'More options' button(when clicking this button shows the custom page in the dialog itself) along with the create button. please help me.
