print the text in textbox to the correct position of the bill slip

Hi,

Im having my bill slip paper in that i have name,amount,date and some other details.

what i need is i have to fill the details(name,amount,date) in textbox and if i give submit the details filled in the textbox should be printed in the bill in correct position i dont know how to do tried all the code in net but cant get that plz any one help.
