How to reuse a page with 5 dropdown list

Hi friends,

I am using VS 2005, framework 2.
I have a selection screen of report.It contains 5 drop down list and a button. This design should be used in 10 report.
Each report is called separately in the menu.
How to create a single selection page in one place and create an object of that page and use wherever it is necessary?
