how to display the text in the popup using dropdownlist

When i click the Dropdownlist1 in that select Poor from the Dropdownlist1 pop up screen will open. the pop up screen as follows

please give your suggestions
textbox1 (In that type the reason for poor)

And i have one ok(Button) when i click the ok button, in gridview, the dropdownlist and reason for poor(in textbox1) will be displayed.

similarily when i click the Dropdownlist2 and select the poor and type the reason for poor (in textbox1)

In gridview as follows

Dropdown Reason
0 not good
1 not intersted


When i select the Dropdownlist1 (0) and that select the poor pop up screen will open in that textbox i want to display the not good


for that how can i do using csharp.
