HTML form for sending email with attachments

I want to use a form in 'contact us' page. It should include email address, subject, attachment and text area for message details.

Please help me as soon as possible

This is idiotic. We'll help you when we feel like it. No-one is going to drop everything and say 'I've got a deadline, but deshmukhvilash needs to know some HTML and doesn't know how to use google'.

The only way to upload a file is with a file upload control, and there's no way in HTML that I know of, of making a form attach a file in an email. You'd need server side code to do that.

Go ahead and define a form with controls that can take email address, subject, attachment... Place a send button at the bottom. Use that button even to trigger the email. You would need email settings in your application for it.
