As I'm a fellow fossil using VS6, I may be able to help.

Bring up the icon in the resource editor. You'll see a small picture of it, and a zoomed in one just to the right for editing.

As has been said, an ICO file has several different sizes in it. If you look above the smaller pic, you'll see a dropdown labelled "Device", and probably "32x32x256" in it. Just click the drop down to see the other icons in the icon file.
