Microsoft Outlook used to work fairly well at detecting and rejecting the trash, but lately it's failed completely to catch garbage, despite the rules I've established. Ten years ago or so I tried out a variety of spam blocking products; all of them were seriously limited in performance, and none were worth the trade-off. Has that changed? Is there a reliable, effective anti-spam product available today?

I think our situations are different, but I had the same problem about a month ago. Out of the blue, I started getting vast amounts of junk.
I run MS Exchange from home, and it turns out that it had something called ForeFront running alongside it which acts as a spam filter, but that was only a 90 day trial. End of trial and it turned off.
That product seemed very good at sorting it all out, the caveats being you need to run Exchange, and also I think MS have abandoned it now. I'm looking for a solution to.
Actually, it would be a very easy thing to code up, if only you could plug your own spam filter in somehow.

It would be nice if Outlook would let me set a rule to block entire domains, but I've tried it a few times with wildcard characters for host names ( ie, *.ru ) and it doesn't work. It seems that there are no longer any standards for TLDs these days,too, since all of this new crap is coming from domains that never existed before, like .party, .science, .xyz and such. Since no legitimate email has ever used such an address, being able to block entire domains would be very handy. Grrrr...

Most effective anti-spam product is deletion of email accounts. Often not an option. Spammers keep evolving their signatures so as to last longer whilst they wares are determined to be spam. How else to manage it, then?
I use a domain catchall to forward all of my mail. Everyone I do any kind of business with has a different email address for me. If I start to get spammed, I know who sold me out because, essentially, their name is on it. I create a genuine forward for that email back to them and problem is solved.
Example: you buy bozo.org. Create a catchall account and forward them all to a nice new clean location that is not being spammed (as it's new). If you do business with Amazaon, the use amazon@bozo.org; similarly newegg@bozo.org, etc.
In addition, I never give out the real email target to anyone (not even family) as it makes it simple if I wish to change my account: change my various forwards and I don't have to bother updating anyone else.
Nothing's 100% effective in that some spam generators use randomly generated addresses and simply hope for any number of hits. It's worked well for over a decade.
