I pretty much just start coding as the image forms continuously as to how I'm going to where I'm going - but that includes commenting as I code. Also, as the code forms, the route changes.
Everything's kind of all done at once - I've done this long enough to where much is implementing mental background processes whilst I enjoy the fun part in the foreground.

Finally code is the heart and surely Actual code will tell you more than documentation or other options.
for e.g. code is the expression and documentation is the words, and obviously expressions speak louder than word

Also, the documentation might tell you the application does x while the code (intended or unintended) actually does y
Code doesn't lie, although it may be very much obscured by bad practices

Code says what it does but not why and there's where documentation is invaluable. Because the next time the code should be modified to solve some nasty bug or implement compatibility with a new component or redesign its behaviour it is of paramount importance to know why things are like they are, what prompted the choice of a certain class hierarchy instead of a seemingly better one, why (if there is a reason) some methods are called in a certain order and so on.

I almost prdouce more sketches, designs and notes than actual code. The times I spent little to no time in pen and paper because "we had to be fast" the projects ended up taking ten times the time they'd taken if I'd designed them properly.
