how to bind id,name ,img from database to template in my folder

actually iam inserting id,name,img using file upload ctrl to database
what i want is for ex if in my company there are 300 employees for every employee creating seperate template is difficult so if i click on submit button values inserted in to database and also id,name and img should come from databse to my template
