Solution 1

All I know is the SDKs by LeadTools and Accusoft. Unfortunately, it looks like they are proprietary and commercial.
Did you see them?

Yes they're commercial allright, but I evaluated LeadTools 2003 especially for DICOM purposes and found they were excellent. I haven't touched DICOM since then so I can't say anything of that API's current state of fitness for that purpose.
