Hi,
I am developing a web application for a client. Shall I ask the client to sign on SRS before development? Or Is there any other document to sign off with client?

Do I need to give screen shots of the form designs in SRS?

Please refer any good site to get a sample SRS.

Those are not simple questions. Disagreement on the requirements with a client is an eternal source of problems. Try to settle at least key things before getting to the rest of the work. If you try to sign all the details, screen shots, etc. you will have to do a lot of work before you get any signed agreement, with a risk to not get paid for it in case of unresolved disagreement... and so on. I don't know a final answer. I depends on many factors and takes experience.

Documentation is a contract between you and your client.

Supply as much documentation as which would make your client happy and as little on your behalf that makes your project economically feasible.
