Sorry about the Full Code.
But you should, devide your file (no matter what file, images, videos,...) in four peaces:
one file name. two file type. three file size, four file content. take the file as a binery object for number four. each of them will be given a special field in DB. to retrieve it you need to assemble them.

you can also use PHP Manual for the functions.
