How to create task in Outlook

The project type in visual studio that i am adding is Outlook 2007
add-in.

I have created a custom task say Nidhi task under My Tasks in outlook by running C# code.

Now i want to anthenticate logged in user when he clicks on custome task i.e Nidhi task by propmting a windows form or some kind of pop up having username written already(that i can do)

with a textbox where user will enter password and a button that will submit the password.

Basically i want on click on custom task window should open.
