Probably Safari on iPad. It seems to be specific for some comments. Maybe because some comment in that solution were deleted.
The same issue occurs also with Edge browser. If a edit comment, in that solution, a bunch of editors are opened.
Same thing with Internet Explorer 11. Same thing with Google Chrome 48.
The problem really seems to be specific to some cases. In fact, it seems that the problem only occurs if I try to edit either a deleted comment or a comment after the deleted comment whatever the level.
If we don't pay attention because other editor are below in the page, we can easily overwrite the wrong comment even from someone else...
I just tried it again with 4 different browser and get the problem in each one. Try to edit any comment in Solution 3 from the first deleted comment and you should see the editor opened once for every comment starting at the deleted one until the last comment in that answer.
I tried to find other questions with deleted comments (except last one on an answer) but it does not happen very often so at this point I was only able to reproduce the problem in that answer.

No reason to close it. If you close this you have to close any article related on this and that Driver.... e.g a MP3 Encoder, for which usually no source is available.
Only my mind.

And enclose it in a code block.
It displays as:

If you edit it, add a space between them:

Then edit it again, and remove the space:

It works fine...
At all stages, the preview shows you exactly what you have typed, but the actual posted answer may be different.

Another one.
When using the hash character as first one after a pre tag, there must be a space or line feed inserted. Otherwise it will miss the hash in the final post but not in the preview.
This will show up without the hash character in QA (here in the forum it is OK):
<pre>#define SOME_DEF 0</pre>
define SOME_DEF 0
This will be OK.
<pre>
#define SOME_DEF 0</pre>
#define SOME_DEF 0

When you respond to messages in forums you can uncheck the "Use Markdown formatting" checkbox at the bottom.
I don't know if it's global and affects also Q&A (where I haven't seen any such checkbox), but it's worth trying.

I was just reading over my article in an attempt to clean it up some and I noticed that an image was missing.

Bad Root Path
I did an inspect (in Chrome) and checked the image name which is UnRio17.png.
Chrome offers the absolute path to that same image as:
http://www.codeproject.com/Articles/1072781/unrio17.png -- if you navigate to that path, you see a CP error.
Finding a Good RootPath
I examined another image in my article and saw the rootpath is probably supposed to be:
http://www.codeproject.com/KB/aspnet/1072781/
I took that rootpath and pasted on the name of my image that wasn't displaying and voila the image loaded:
http://www.codeproject.com/KB/aspnet/1072781/UnRio17.png[^]
I also tried a refresh on the page to see if somehow those rootpaths are generated and would generate properly, but the image never shows.
I hope this helps make resolving the issue a bit more easy.

As mentioned by @Nelek , It makes sense if the protectors can get a visibility of members who have approved some dubious articles.
@chris-maunder mentioned that little education should solve the problem. In such case if the protectors know who has been careless with the approval system, they can explain them the process
