The reason for adding the comments as a "Solution" is to remove the question from the "All Unanswered" list. The original message most likely has already been answered.

That was my fault. I accidentally reported your answer instead of another's and when I saw my mistake I went back to undo it but couldn't find your answer. I meant to email you but was hit with 10 distractions from all sides and totally forgot. Sorry mate.

People more and more use the online Fiddle to prove or show code.
Perhaps a collaboration with those guys would be nice to allow some sort of "fiddle" integration in the forums.
basic idea
eg. If you press the "Fiddle" button you're presented with the fiddle of the correct forum (javascript, C#, ...) a button on the Fiddle forum allows the code to be copied in the answers with the correct formatting and pre tags. You could also select the code from the question itself and send it to fiddle.
just a thought.

Whenever the title of a thread changes in its answers the Discussion tab shows a new entry, as if it was posted at the top level. Take a look at the thread:

Users like this poster: [^] can repost away without consequence. Note: by mentioning this, I do not mean to imply this user is engaged in any form of "deception."
I think not requiring the name visible is a mistake, and, I do not buy the idea that "rep-shame" is a significant causal factor in high-rep members not posting questions with a similar frequency as "modal other users."
You may not like my saying this, but another possible scenario is that high-ranking posters know it is much easier to quickly find relevant content on StackOverflow because, possibly, said high-reps are not lost in static, know what they need to know. Do CP high-reps post fewer questions in general because of acquired skills in search, and awareness of how to get to select resources ?

I'd say some high-rep users are far too busy throwing whatever-instant-solutions at questions pop into their heads based on their psychic powers ... in order to be even more high-rep ... they don't have time to program, let alone ask questions
Want to improve the quality of QA ? Require posters to tag/state the OS/stack, programming language, etc., before they can post. It is ludicrous to see fine minds of many CP members who generously devote time to QA ... again and again ... having to "beg" posters to at least tell them what language, or FrameWork, the question is about.

To address your concerns
1. We need a better way to identify and link reposted questions. It should never, ever rely on your having to remember that a member posted the same question previously, or to search manually. Our system should find the issues for you
2. A question should always be reported on on its own, regardless of the author. Once you report a question the author's name will appear in a tooltip over the report widget. Further, I'm making a change so that question posters' names will always appear to moderators.

Gosh, I wish I could blush  ... no one ever totally agrees with me. The gist of my many suggestions, over years, is:
1. before a member can post a new question on QA (and the language discussion forums ?):
1.a. a UI of some type appears with some controls that require the poster to specify the language used, the .NET stack used, FrameWork version, etc.
2. ... a more "radical" idea: if an MVP or Mentor responds quickly to a new QA post with a comment that the post as-is is ambiguous, unclear, etc., and asks the OP specific questions:
2.a. while those questions remain unresponded to: block the thread so that posting solutions will not occur however, allow further comments on the OP.
Admittedly, the second idea is probably a no-go in terms of CP culture, and could create problems: rare folks like OriginalGriff who have awesome abilities to sniff the real issues in jumbles of verbiage and shards of strange code should, of course, never be constrained.
May I suggest we somehow have a pow-wow (Mentor and MVP forum ?) to try and harvest our collective wisdom around improving QA ?

At the current time that's the only method we have and it has effectively been removed. 9/10 when I report a repost it's because I think "that kinda sounds familiar, I'm sure someone previously suggested XYZ but there are no replies to this", or I have some other vague suspicion. By clicking the user profile and checking their history I can verify it is a repost 100% and report it. This can often involve comparing the two questions to see if I think the new one contains enough difference to warrant a repost. With this update it takes more detective work than I'm really prepared to do to find if something is a repost.

There is no "rep-shame" for me, I have only 3 questions because I found answers to my other questions by myself.
I am a dinosaur coming from an era before internet, at this time the main help was documentation and debugger. So the answer was self studying and experiments.
As of today, I have enough experience so that when I Google, I am able to extract useful partial answers from what I find on internet to build my own solutions.

Have y'all noticed that when you reply to a comment on a Q/A question, your reply appears as if it was made to the wrong comment. Hitting F5 to refresh the page fixes it, but it's causing people to delete and re-reply (only to have the same thing happen again.

Looks like you guys need to do some image massaging in terms of dimensions.
On my own websites, uploaded images are reduced (at highest quality) to a specified dimension (if necessary), and then compressed until they are <= than a specified file size. For example, regardless of how large the uploaded image might be, it is store at no larger that 640x480 and consume no more than 75k on the disk.
Having said that, I think the image should at least be in a div with overflow set to auto.

Click the spam flag, and wait for the shield to appear. Hover your mouse over the shield and a link to the user will be revealed in a small popup.

The sign in page is broken.
You cannot enter in an email address when you want to sign in.
I tried in both Chrome and Edge.
I had to use a new email address to be able to get here.
