How to Create a Application Form That Need Approval At Multiple levels

I am new to this field and dont have any experience
I need to design an application for organisation carrying out training for Students.
Organisation have different Schools.
Each School has
Following Approving Authorities
(a) Principal
(b) Wise Principal
(c) Head Clerk
(d) Library In charge
(e) Maintenance In charge
(f) Mess Secretary

Now when a student need to proceed out of this organization he has to take clearance from various authorities as per movement type. The various type of movements are:-
(a) Routine Holidays
(b) Temporary For Sports
(c) Permanent on Completion of Training

In case of (a) & (b) type of movement clearance will be given by the approving authorities mentioned above. However in case of permanent transfer new authorities will be added which are as follows:-
(a) Lab Incharges
(b) Canteen In-charge
(c) Ty Fund In charge
(d) Instrument In charge
(e) Tools In Charge
(f) Sports In charge

The application demands as follows
(a) Individual Puts his Request through a login page
(b) The approval page should be available to concerned authority and not all.
(c) It should show the status of individual approved or not
(d) If not approved,remarks
(e) None other than concerned should be able to approve the request.

You can do it by appropriate software design and implementation work. Not clear what help would you expect for us. This is not a good question. Imagine that you are asking: "how to skate"? Do you think there are answers which can make you a skater? Perhaps one: "Just do it". Your question is pretty much like that. Care to try? :-)

Hi,

Since you've said you are new in this field, maybe the appropriate question is "Can you provide me some tutorial sites to help me on making my application?" or "Can you suggest some helpful books and where can I download them?". This site may help you. www.w3schools.com[^]

If you already know the basic, then it's time for you to make a plan on how your Application Form will look like. I'll give you some guidelines to design your Application Form.
0. Your Application Form (AF) must comply to what you really need. In your case, you just specified that above.
1. It must be readable and easy to use. Or what we call, user-friendly.

Designing requires a lot of understanding and logic of what you want to achieve.
So, basically, we can't dictate you on how it must be designed.
I suggest you try to self-study on your chosen programming language and plan for your AF. If you're stuck when you start coding, then you can come back and ask questions. :)
