get real - we don't do homework - and there must be plenty of examples out there on the web - make google your friend

at a first look, you seem to have 4 'areas/topics', so you'll need a number of data fields/items for each - start mapping it out on paper, eg

Personal Details
First Name
Last Name
Date of Birth
...
Education Details
highest school
...

- you may want to either split your page up, or use tabs for example for each 'area/topic', to not put too much info on the page for the user
