but i need to include user defined screens during installation.

where do i get the documentation?

isn't deploy the form to exe called compile?? just wondering.

If you're using the setup projects of Visual Studio, I´d recommend to you to read the documentation, they have examples on how to create screens that do what you need and how to "link" them to an installer class inside your solution.
