At this moment, I would only advise Visual Studio. Yes, I really think that manual design writing HTML code directly gives the best results. Having a debug HTTP server makes the feedback and debugging faster. Not only you get maximum flexibility and quality of the final result, the development can go faster than with all fancy designers, because you have all you need in one tool, from code to deployment to testing.
